Application Form:
Necessary application forms and prospectus for admission to various classes can be had from the college offlce on payment of Rs. 35/- at the counter or by sending Rs. 55/- for obtaining the same by registered Post. The M.O. Receipt must be sent along with the application form.
A self addressed unstamped envelop (4" X 6") should be submitted along with the form for despatch of the intimation card by Regd. Post.
Index:
Index Card will be given at the counter to the candidates after the receipt of the application. The candidates should quote the Index No. in all correspondence with the Principal.
Intimation Card:
Intimation Card will be sent only when the candidate is selcted for admission.
Important:
1) Admission cannot be claimed as a matter of right.
2) The decision of the Principal is final.
3) Merit and conduct shall be the most important factors for
admission.
4) Late applicants shall have no claim for admission.
5) There is no provision for admission by spot selection.
6) Admission forms should be completely filled in and the
following documents attached to the application form. The duly filled in form with all requisite documents should be
submitted/sent to the Principal ( so as to reach him on ".

Attested copy of S.LC. or C.LC,
Attested copies of Mark sheet of the qualifying
Examination,
c) Attested copy of Conduct Certificate from the Head Master
/ Principal. .
N.B :
Those candidates who have passed prior to Annual Examination, 2006 and non-collegiate candidates who have passed the qualifying examination privately should submit attested copies of two separate conduct certificates from two different gazetted officers.
d) Money Receipt of the application form.
e) Attested copies (two) of Provisional Pass Certificate (for P.G. and Private candidates)
f) Attested copies (four) of RECENT passport size
photograph, one along with form and three other copies required at the time of admission.
g) An attested copy of case certificate in case of S.C./ ST candidates granted by a competent authority / M.P./ M.LA Collector/Add!. Magistrate/Sub-Divisional Officer/SubCollector of the area.
h) i) Attested copy of H.S.C. Certificate reflecting date of birth (for +3 1st year & 5th year P.G applicants).
ii) Attested copy of provisional Bachelor's degreee/+ 3 certificate in case of 1 st year P.G applicants.
iii) Attested copy of mark sheet of +3 degree
examination including back papers, if any, in case of 1 st year P.Gapplicants.
i) Attested copy of Migration Certificate for candidates who come from other University / Board / Council.
j) a) The application form should be submitted directly at
the counter on receiving which the index card will be issued immediately. b) The Intimation will be sent by Registered Post.
k) Specific mention should be made for the following .on the top of Application Form in Capital letters as applicable:
SC./S.T.-for Schedule Case/Schedule Tribe; Hostel for candidates who wish to reside in hostels; 'W' - for women candidates; 'O/H'- for Orthopedically Handicapped candidates;
Ex - Serviceman -for children/wife/husband of ExServiceman; and Bichhinnanchal - for Oriya Students living in Andhra Pradesh
Attested copy of medical certificate for orthopedically Handicapped candidates from Chief Distric~ Medical Officer. ."
m) Attested copy of Discharge certificate and service particulars in case of ex-serviceman.
n) Attested copy of Nativity Certificate from the local Revenue OfficerlTahasildar of Andhra Pradesh in case of Oriya living in Andhra Pradesh (Bichhinnanchal)
o) Attested copy of "A" "B" "C" Certificate of N.C.C., President's Guide/Scouts/Sports Certificate of state and Inter- University level if any.
p) Xerox copy of any document submitted should be attested by a gazetted officer.
q) Full present .and permanent address of the cand~date should be clearly and legibly mentioned at the appropriate places in the Application form
r) Admission into Post Graduate classes will be by pus tal admission. ...
7) Applications incomplete in any respect will
be summarily rejected
8) a) Admission into different Men's Hostels will be made at the time of admission and shall be on merit basis.
b) The Vacant and available seats in the P.M.G. Hostels 1& II for women will be first alloted to women candidates belonging to S.C., S.T. and ifseats are available thereafter, fhose would be alloted to general women candidates on
merit basis.
9) Candidates selected for admission into different hostels . have to give an undertaking. The Parent /
Guardian as indicated in the body of the Application Form shall accompany the candidate in
person and sign"the undertaking in the presence of the Professor in Charges of admission or
Superintendent of the hostel concerned.
RESERVATION;
Scheduled Castes and Scheduled Tribes (SC & ST)
i) There shall be reservation for SC and ST students tthe extent of 8% & 12% of the sanctioned
strength of 1 ST Year classes respectively. The reserved seats are not interchangeable.
ii) A student seeking admission under SC/ST category shall produce caste certificate from the
Local Revenue Officer not below the rank of Tahasildar/Local M.P.! M.L.A in Proper.

Physically Handicapped (P.H.)
i) There shall be reservation for Physically handicapped students upto 30% of the sanctioned
strength 'in 1 st Year classes.
ii) A student seeking admission under "PH" catgory shall produce a certificate showing the
extent of handicap to be 40% or above.
c) Burma repatriate (BR) .
2% of the sanctioned seats shall be reserved for children of Burma Repatriates.
A Certificate from the Rehabilitation Officer in change of Burm Repatriates should be
submitted for admission under this category.
d) Ex-Serviceman (Ex.S)
1 % of sanctioned seats is reserved for children wife/husband of Ex-Servicemen. Certificate from competent authority Rajya Sainik Board/Zilla Sainik Board shall be submitted admission underthis category.
e) Defence Personnel (DP)
1 % of sanctioned seats is reseved for the children of serving Defence personnel. A certificate from the Commanding Officer of the unit Concerned should be submitted for admission under this category.
f) Freedom Fighters & Martyrs (FFM)
2% of seats sactioned are reserved for the children of Freedom fighters and Martyrs. Certificate from competent authority should be submitted for admission under this category.
